Sail training vessel “Picton Castle”:
In 2006, Laura Gainey, a 25-year-old crew member, was swept overboard, during a storm, when the ship was bound for St. George's, Grenada. An investigation was done by the Transportation Safety Board, which found the decision to sail, given the forecast and the lack of training among the crew, contributed to the accident. There were no accidents before, and have been none since Gainey's death.
The report identifies seven areas as contributing factors, related to Gainey being swept away. They include:
The decision to sail despite the long-term forecasts of an adverse weather system.
Insufficient communication between the officers and crew.
Fatigue and loss of alertness among those onboard.
Safety nets not being rigged in some areas of the ship.
Safety harnesses not being worn.
