Bad luck for the clipper crews.
It would have been a nice detail to know if his PDF / Harness came off or the tether / jackline broke. I'm sure a detailed investigation will be made.

Sad news. Hopefully, the investigation will reveal what went wrong with the the tether system and we can all learn from it.
