As steamgoat said in post #6 zephyrwerks or something like that can make exactly what you need for very reasonable money. We have used them at work on customers rigs.

selden has many sizes of sheaves available as spares, there gotta be some that fit. look at selden spares (e.g. boomspares) catalogue, the sheaves are given with their dimensions
