Crew List Forms?

Anybody know if there is a specific form for a crew list? We are going to Mexico in a couple of weeks. I was thinking just a typed list in MS Word on ship's letterhead.

latitude 38 has one, hete has a crew list--use search feature here for crew list...
google latitude 38 crew list. it should show up in htere. some good souls in that bunch.

Downwind Marine in San Diego used to sell the Rol de Tripulantes forms, don't know if they still have them, but once you have one, and scan it, you have as many as you need.

We have been using the one Eric on Sarana created for the fleet a number of years ago. We just type in all the stuff that doesn't change and then make a bunch of copies
