Wooden Name Plate Stern of Boat

i have a 41 formosa and i am looking for someone who can make a plate to insert boat name . it had two wings on eac side and is about 4 ft long open or flat in the center to put name....it also had a smaller one that was made the same with the home port name... under 2ft longs looking for any help.... thanks......can email pics of what i am talking about.....
