Just buy the screw you want and a plastic/nylon "standoff" of the length you want ...available at any hardware store.
That is what it appears to be. There are shoulder screws but that doesn't look like one.

I think what you are looking at is actually 3 pieces. The first piece in the cap base and is put on the screw first, The next piece is a plastic (nylon) sleeve or bushing that is slipped on the screw next. Next it is screwed into place and the cap is installed last. The nylon bushings are available from almost any hardware or home improvement center. Some will have the caps and bases as well. When I had my upholstery shop I bought the caps and bases at ProDec and they have a large range of colors. Hope this helps.

They use the same things on RVs to tie off the lines that raise the shades. I had them on my motor home and had to replace several of them. I got them at Camping World but I have seen them on ebay and Amazon.

They are called Canopy Buttons at Whitworths in Australia, a nylon button that has a central hole to take a screw, intended to take a loop of shock cord or similar around the button.
