Interesting report into the grounding of the "Oliva" on Nightingale Island.

The lifeboat mentioned in the report eventually came shore at Salt Creek, South Australia last month, after some 99 weeks and a 5000 mile voyage

I found it interesting, that ‘Parallel Indexing’ (PI) was not part of the company’s standing orders in passage planning:

This would have assured that the ship had the proper chart properly prepared for clearing distance and would have highlighted plotting error.

Also with both X and S band Radar’s, the island should have jumped out at them in those moderate conditions if they had been using S band for PI

Absolutely correct on the PI, its such a simple method and immediately gives an indication of the vessels track.
What I found the most telling, was that no one seemed interested that the voyage would take them close to the islands. I did a similar voyage when on cargo ships, and it was like kids in the back of the car "are we there yet". Passing a group of islands was something that broke up the monotony of the trip and the ETA of passing was pretty much ingrained.
Still, at least it looked like the owners did the right thing after the grounding.
