I'm facing the same questions with my 40 year old Cheoy Lee, and roverhi's comment that he "can't remember hearing of any that have lost steering because the rudder shaft is turning inside the rudder blade" struck home. Has anyone here had first or second hand experience with this type of failure?

Nope, one that was loose in the fiberglass shell though.
