A blind rivet. You can also drill and tap for a machine screw, but unless the mast is down, an accurate tap is harder to do.

No block needed. A small u-shaped eyestrap riveted to the bottom of the spreader works great. The small amount of friction is no big deal with 3/16 line running through it.
