Supplemental Small Boat Experience Letter

Hello fellow sailors. I'm working on getting my OUPV from the USCG. Anyone have a sample Supplemental Boat Experience Letter that works? Also, how can I "log" time for prior USN service? Who authenticates that if I was TDY/TEMDU on ships rather than formally attached (in other words, by SRB won't reflect the ship as formally being a command I was attached with). Thanks!

there is a small sea-service form that you can download from USCG National Maritime Center website,,,,

there is also an email on the front page of the web site that say iasknmc or something like that,,, they usually reply in less the 48 hours,,,the rec's no longer have much to do with the licensing everything comes from west virginia at nmc headquarters
