In my experience you should plan for a passarelle of min. 3 m in length attached to the furthest aft point you can manage on your cat. This will enable you to pull the boat a bit further...

Provided you and your crew have valid Schengen passports there is no need to clear out of Italy as Malta is also a Schengen country. Just sail across!
