USCG deletion from documention?

Would anyone have a doc / pdf / template / form for a USCG deletion?

I helped my friend thru this process. He is not a US citizen so he registered it with the state. All he did was have the seller right a letter to the USCG requesting deletion.

Dear sirs, I am requesting deletion etc. will do.
Send it in with the documents.

Also the seller will have to write that letter and furnish the documents.
