Deletion from documentation questions

I need to delete a vessel from US documentation and the instructions the coast guard gives for this are confusing (http://www.dco.uscg.mil/Portals/9/DC...-09-113141-770)

Is there anybody who has been through this process and can answer the following questions:

(1) Can I do this in person? If so, where?

(2) Where is the "fillable form"? On the NVDC web page I could find no form for deletion of documentation.

(3) What documents do I need? So far, my best best guess is:

- the existing two-sided certificate of documentation
- a copy of the bill of sale (one page)
- a "letter" requesting deletion and/or a form (??? what form ???)

(4) What is the fee for process? Is it $15 for the request plus an $8 fee for filing the bill of sale, for a total of $23 or is that wrong?

(5) I need a certification of deletion from the coast guard, if I do this in person will they hand it to me afterwards, or will I have to wait for it to be mailed to me? What do I do if I never get it?

I eventually called them and found out the answers:

(1) You cannot do it in person. You have to do it by fax or mail.

(2) There is no form.

(3) If the back of the certificate of documentation is filled out as a transfer, and is signed and notarized, then you do not need a bill of sale. You can fax them the document immediately and mail it later. If there is no transfer filled out on the back of the certificate, then you will need a bill of sale.

(4) If you are have a filled out certificate, then you just pay $15. If you have to submit a bill of sale, then you need an additional $8 for every page in the bill of sale. If you would like to have the certificate of deletion emailed to you, then include an additional $4 fee and request that in your letter.

(5) They mail you the certificate of deletion. You can get it faxed or emailed to you for an additional fee of $4.

It takes about 4-5 business days to process the request.
