The big metal pipe (maybe elbow shaped) conducting the raw water coolent out of the heat exchanger and the combustion products (exhaust) into the sea could be sufficiently blocked to cause the pressure build-up in the heat exchanger to force raw water into what should be a closed anti-freeze circulation system, thereby increasing its volume and causing overflow. This blockage could be sufficient EVEN THOUGH you might see exhaust and plenty of water coming out into the sea. Ask the people who serviced your heat exchanger last year if they changed this part, i.e. the metal joint (maybe elbow- shaped) from the heat exchanger to the rubber tubing which conducts water and exhaust to the sea.

How is this Possible?

Re coolant in hot water.
I believe you can taste and smell coolant long before it is at a toxic level, and who drinks hot water anyway?
