Boat Stamps

OK, this might fall under the category of "don't worry about the minutia", but I'm wondering if any of you have produced a boat-stamp (the inked, rubber type) for your vessel, and if so, what info did it contain?
We will be actively cruising soon, and I have heard and read that beaurocrats in other countries appreciate the stamps, and that they can make checking-in, and other official endeavors, go more smoothly.
I have worked up a prototype, with the following info:
Vessel name
Documentation number
Owner's name (or should it say "captain" instead?)
A signature, and date, line

Anything else?

In an eleven year circumnavigation, I used our boat stamp about three times. I don't know what would have happened if we didn't have it. I suspect nothing would have happened, because others made the same trip without them.
