What do the diplomatic initiatives from Washington and Havana mean for cruisers? Is there any sign that the effects will mean new opportunities for boat travel and a change in how the US Coast Guard deals with those suspected of violating the US embargo?

I'd say you still have a bit of waiting to do until normal tourist travel is approved...

Perhaps the first legal opportunity for US residents to visit Cuba by boat will be if approval is given for organized events such as rallies... might see something in a couple of years, perhaps.

It's always been easy to get permission from the Coasties to go to Cuban Ports for repairs when needed ! Just sayin sometimes things break at the darndest times !! LOL
