I created what I though was one route around the tip of Michigan.

Open CPN sees 5 routes.

I asked it to Extend in an effort to combine into one route, but no luck

How can I combine these 5 routes into 1?

You have to mark the 5 routes in the Route&Mark Manager, then export them into one file. Open the file in a text editor.
Each route starts with <rte> and ends with </rte>. Leave the ones at the beginning... and in the end of the file. Delete the other ones. Don't touch the route points, <rtept> or anything else. Then import the edited file back into OpenCPN.
A bit of a fiddle, depending on what you are used to.....

The easiest is probably to just recreate the route.
