I guess, you can adjust your coverage while sailing to the different regions you intent to be. I would start with a coverage for the Med / Europe and when preparing the passage get covered for the atlantic, then caribbean etc.

Could be much cheaper and you buld up some reputation in your skills in this time, so the rates cold be better than having worldwide coverage from start.
