We got ours when we booked charters through the folks we booked with; same for
classes. Is it necessary? Well, it sort of depends. Consider the following:
You're at the hospital because a close family member is critical condition.
Your charter is leaving on a Sat morning but you won't get there untill Sat night because you are driving or your flight is delayed. If the charter is just you and it means you end up with a day less sailing, that is one situation. But if you are chartering with two other couples -- the boat is leaving and you missed the boat.
If you are willing to pay for the trip anyway then you don't need the insurance. If that would be a real pill, you're going to want it -- if only for the peace of mind.
As for the cost and what it covers, you're going to want to read the
contract.
