Captains Health exam

I am in Jacksonville Fl and I have to take my Captains License renewal, health exam. Any one know where I can get that done here?

Any MD (& others as listed) can perform your physical exam.
The drug test must be performed by an accredited lab (see lists).

I know that any MD can give the exam but what will they charge.
I had an exam here 5 years ago by someone that it looked like thats all he did
and it was very reasonable.

I suggest finding an agent to do the whole renewal process for you (although my guess is that most of them are in Miami). They work with MD's and Labs who do the test to USCG standards all the time. I had to renew mine a few years ago, post 9/11 it is a huge hassle. I hired an agent and was very glad I did. Made the whole process much smoother.

I just renewed my license. All you have to do is to go to your own licensed physician and have him or her fill out the GG 719K form at the time of your physical exam. Make sure ahead of time that your physician has the device for the hearing test and the cards for the color blindness test. Why they test for color blindness each time I don't know because that does not change. Look for the licensing wizard checklist on their site.

I did not need to hire an agent to do all the paperwork for me. Although there are a few hoops to jump through, it is not very difficult. It is time consuming, yes indeed.

You need to give the CG up to 90 days to review your medical form after submitting all of you license application paperwork. At the very least start your renewal 4 months before your license expires and start your first license application process 6 months before you need your license.

The CG could not recommend a certified medical review officer to approve the drug test itself...only the drug labs that are certified by the CG...hopefully that makes sense. Google was my friend in finding a medical review officer....which has nothing to do with my medical review, just the drug test. Yes, that part is a little confusing if you have not gone through it before.

The Coast Guard is really good at helping you on the phone if you have questions, which I did because things change with licensing each time I go to renew. They seem to add a hoop or two or change the procedure for doing something each time I renew. But that's okay and understandable. Their requirements are spelled out usually very clearly in their licensing website.
