Sextant Worksheet Question

Hello Everyone,

I have been racking my brain and have been searching the internet trying to find out where I saw a tri-fold laminated 3 page ( one left side and one right side ) folding to the middle sextant work sheet cover.

It's the kind you put a blank piece of plain paper in and fold the left and right sides twards the middle, then you follow the directions printed on the page (s) and are able to find your position.

Anyone out there who can help me find a source, I thank you in advance, Cheers Ron.
