OUPV RENEWAL

Struggling to renew my OUPV/ six pack. This question comes up on a couple of forms. They are very clear that if you don’t answer the question the application will be rejected.
Anyone here have a clear idea on what they want for a simple OUPV application? I’m guessing “OTHER” and then write in OUPV.

For my recent 100-ton renewal, I entered something like:
"Same as previous: Master etc etc etc etc etc"
IOW, I copied the endorsement from the previous credential.

I highly recommend the online chat feature that the Maritine Center offers. You can get your answers straight from them. Since it's just a renewal it should be pretty easy once you learn your way around the form. Make sure you use the checklist for your renewal. Itll have everything you need to help it go smoothly.

Are you using Form CG 719B with the name OMB 1625-0049 in upper right corner??

I didn’t get email notices that anyone had replied to this thread. Sorry for not responding.
I did find the online chat tool and used it. Worked for me.
