Probably part of his wetsuit hood. The first responders would have cut it off of him, leaving that piece so as not to disturb the shaft of the spear.

Judging by the background of the pboto, the guy is in a hospital bed and surrounded,by medical equipment. The rubber tubing is air or oxygen fitted to his nose......
Have to rename him LUCKY!

The shaft is the only thing sealing the blood vessels. Pulling it out would release a torrent of blood. Good luck .

When I was an intern in Philadelphia some years ago we had a patient brought in with a Javelin through her head (she was wandering around during track and field practice). It was a wooden javelin so the EMT guys cut it off on both sides of her head so she could fit in the ambulance. Then we did a CT to confirm that it hadn't gone through any major vessels (carotid, jugular) and then it was pulled out. She did fine...just like this guy
