I have seen that happen too. When they put the two halves of the sling together with that giant pin they forget to put the safety bail/wire (they are much like a sewing safety pin but huge) over the end of the pin. Then the pin works its way out of the joint where the two halves of the sling are joined.
- - I always watch them join the slings halves and make sure the pin is captured by the safety bail/wire.

There was a very informative article in PassageMaker magazine a few years ago about a NordHavn that was dropped, how they proceeded, and I believe they were "made whole" by the insurance companies involved. The strap broke on the travel lift and the rear of the keel punched up into the engine/equipment room.

It might be worth referring to the article. Certainly people need to be aware of their insurance coverage in these situations and also what liability the yard takes.
