Hi Grunzster,

Is the certificate you were given a 5 x 6 piece of paper ...titled

" Application for a Boating Safety Certificate" ?

If so, it should have the name of the Agency...Course administrator
The name of the administrator and the name of the instructor on it....and the date of the class....
