Perhaps there's nowhere like this left but I'm thinking about a place where people are as likely to be drinking at 6 am as 6 pm, where the bartender knows the local waters, where a deckhand could find work or a place to sleep etc.

fair enough I don't get over there as often as I'd like so my experience is a little limited, I've been on the east coast of Asia since I turned 21 so i might not know exactly what I'm talking about.

Being a Florida Boy, I have to second Schooner Wharf in KW, and Alabama Jacks in Card Sound. Then add Casey Key Fish House in Osprey, Rotten Ralph's on Anna Maria, and the Black Dog in Cedar Key

The Oar on Block Island is perhaps a bit more touristy than salty, but the sailboat racing teams hang out there, as do all the other sailors, who can sit on the porch or on deck chairs on the lawn and drink beer while watching their boats sit comfortably on a town-owned mooring. Anchoring is possible, too, in the northwest corner of the Great Salt Pond.

The food is good at The Oar. The dinghy dock is a clustermug.

Some of these are more locals bars than sailors bars, but

Brooklin Pub (basement of Brooklin Inn), Brooklin, Maine
(something) Street Pub, Rockland, Maine
The Broken Down Valise Saloon, Mattituck, NY
Lennon's, Port Washington, NY
Half the bars in Fells Point, Baltimore...Cat's Eye, 1919, "Bar" (really, it has no name) ... Just go a few blocks from the center tourist bit and walk around you'll find any type of bar you seek...tie up at the free dock at Browns Wharf and stay as long as you like
There's some places that almost qualify within walking distance of the free docks in Portsmouth, VA
A couple of joints in Belhaven, NC
Can't remember the name of the half walled in joint below a building next to the Charleston City Marina

+1 on Davis's and the Backstreet. The bars are out there...sail away from the condos and towards active or run down fishing docks and hoof it around. They're usually a few blocks back from the water, too.
