Hazard to navigation-depth and position unknown

What do others do when find paper or electronic chart notation of a hazard to navigation that only says "depth & position unknown". Ignore if good visibility, avoid by feet, yards, miles?

I'd go with at least 1/2 a mile in good weather a mile in bad.

I did some reading about the "position unknown" notes, and found that many that were later pinpointed were miles from the initial reported position. If you look in the coast pilot for the area, you will see that many (maybe most) were reported long before GPS and many have never been relocated. Use lots of caution anywhere near that sort of notation. Just my 2 cents worth. _____Grant.
