Crew list template? St Vincent & Grenadines

Hello - we will be sailing into St Vincent Grenadines next month. I have read that 3 copies of a crew list are required - what info is required for a crew list? Is there a template I can use and just photocopy to save me writing lots out by hand?

For St.Lucia I have signed up to Clear Sail.

Yesterday I downloaded copies of the shipping articles. Although not a requirement under fifty tons, no one crews on my boat without "signing articles ". Everything is covered, the owner is covered legally, the crew members are covered legally and have record of sea time. They are the same articles merchant shipping uses. Win win and win!
