You can not clear out of the US mainland on a private vessel
Its because they don't know if you actually go. On a plane at an airport they know you go - its all a secured area. Not so on a private boat.
When I arrived in the Caribbean after the USA I emailed US CBP a screenshot of my entry papers, my cruising permit, ships rego so I hope they don't think I am an illegal overstayer

Clearing in here without us exit papers is fine because they know about it.

If you run into that Antigua situation you just leave and go 30 miles to Gaudaloupe clear in there then out and return to Antigua.

If you ever get into paperwork trouble just head to a French island. They are conveniently splattered through the Caribbean

Last time I cleared into SMX by boat from the USA was 2012.. sailed from Ft Lauderdale and had to detour S to the island after an engine room fire.. no hassle.. no interrogation..

The same customs & immigration office that cleared you in will give the (same) form to fill in for outbound clearance; you fill it in with one extra field for departure date and they stamp it and give it to you to present to your destination country's officials. No charge and no problem.

I have never cleared out of the US to go to Canada nor cleared out of Canada to return to the US via boat. Only boated into Canada the last 4 years, but I don't think that rule applies to all US / Canadian travel. Heck, I think both Customs / Immigration groups may operate out of a combined database sometimes. And as MarkJ said, you cannot clear out of the US on a private vessel.
