re: Thinking Outside the Medical Box: Seizure & Diabetic
Lessons to take on your boat...... - KNOW your crew's history
- Download to hard drive or have a book listing what medications do (PDR is excellent)
- Download or get a book that has a LOT of emergency medical info... not all mishaps at sea are trauma!
- Be a detective! Look around and see what you have to work with. "Paging Dr. Mac Giver"
- If you plan to make passages outside the coverage of modern medicine, take a course in emergency medicine (or if you're well heeled, HIRE crew that has the training)
Once again, this scenario is TOTALLY outside normal medical protocol, BUT you have to make due.

I NEVER wanted to be a paramedic.. I wanted to drive the big red truck and squirt water! They MADE me go to paramedic school!
Only if she is under 45!

re: Thinking Outside the Medical Box: Seizure & Diabetic
Doesn’t matter.. MOST EMS agencies require OLMC for epi 1:1000 SC in anaphylaxis PTs over 40-45
In cardiac arrest the epi is 1:10,000 IV/IO and the PT is already dead... whaddya gonna hurt? LOL!

re: Thinking Outside the Medical Box: Seizure & Diabetic
Great teaching moment.
I've been a lifeguard and have wilderness first aid, but after ABC, I did think of raising glucose levels, but got lost beyond that.
Just a suggestion for the next time around, less acronyms for us lay people, and although I know what about half the meds where, perhaps an idea of what meds listed are typically used for.
Thanks again.

re: Thinking Outside the Medical Box: Seizure & Diabetic
Thanks guys, but believe it or not, I've gotten PMs condemiming me for not allowing her to die! One suggested that I am going to encourage cruisers to play doctor. In light of that, I want to say one more time: THE ACTIONS DESCRIBED ARE NOT A SUBSTITUTE FOR SEEKING PROFESSIONAL MEDICAL CARE. THE SCENARIO IS ONLY A LAST DITCH EMERGENCY EFFORT TO SAVE A LIFE, AND IS CONTRARY TO STANDARD MEDICAL PROTOCOLS FOR A SIMILAR SHORE SIDE EMERGENCY!
OK that out of the way, it seems the majority has an interest in seeing more. I'll try to regularly drag one of these out of the closet. The above was a real event almost in real time. I'll see if I can adapt some land side scenarios to the water. I'll do it as a continuation of this thread. If any other medical people want to share a story, I'd be honored.

There is no place offshore for medical secrets. In your scenario (a very good one) the crew member actively concealed her issue.
My point, in support of your take-away lesson, is that the captain/owner/chief bottle washer should have a clear discussion with all crew to elicit any potential issues. There is no solution for dealing with lies or prevarication but the effort must be made.
