Elsewhere, someone swears he has a "no car" liability policy with USAA for only $5 per year. The caveats being that he has to be out of the US for most of the year (IIRC) and of course that USAA does't serve the general public, only past or present armed forces members. You might want to recheck with them.

I can't see why anyone would issue a policy for less than the cost of sending out the annual policy bill...but that's what he said, five dollars, no owned car, no specified amounts on that liability coverage for rental cars.

I have been using ebookers - Cheap Flights, Hotels, Car Hire & Holiday Packages with great results. The price includes all insurance and fees including airport fees, taxes and anything else they can concoct to add to the bill. The only additional fee I have incurred is a one-way drop off fee.
When compared to the cost of renting a car for one week in Key West, for example, the cost was less than half what it would have cost through any of the other travel agencies.
