Health Insurance - Caribbean

We Europeans are normally covered only for 3 weeks in USA ; after that not or against very high premiums.

How do others insure themselves / or not / healthwise when going cruising for long term in that part of the world?

I one of them europeans, but we have mostly private health insurance at home and it covers us for 6 months in the US as standard!

Will your insurer give you extended coverage as a rider, for an additional premium? Can't hurt to ask.
