Do you need to submit the original signed Sea Time Serivice Form?

Just wonder if a printed scan copy of the original form will do when submitting to CG for Captain license???

I didn't even use the form, I just had a bunch of letters from captains, crew, self signed, basically it was a grab bag of stuff. This may have been long enough ago that the procedures were a little looser, but at the time they didn't give me any grief about it.

As I understood it at the time how seriously they take it has a lot to do with what license is being sought. The first OUPV is pretty informal, stepping up to larger vessels is much harder.
